| If your child requires medication to be administered during school hours, a written authorization form needs to be completed by both the Parent and the Pediatrician. A medication consent form is available at the top of this webpage for download or you may contact the Health Office at Hale for a copy. |

Physical Examinations & Immunizations |

Attention ALL Parents (especially 6th-grade!): Schedule your appointments now with your child's physician to be sure all necessary immunizations have been received prior to the start of school in August. This is a requirement for students entering 7th grade. A letter regarding these requirements was mailed last month. (see details below!)
Please contact my office at 978-897-5074 if you have any questions.
Don't forget to make an appointment for a sports physical even if your child is entering 6th or 8th grade.

In Case of Injury... |

If your child has sustained an injury such as
a fracture or a sprain, please plan to meet with Mrs. Baranofsky, RN,
to discuss your child's needs upon returning to Hale. It would be
preferable to know of the injury prior to the student's return to
school. This is especially important if the student will be utilizing
an assistive device such as a cast, crutches, or a wheelchair. Even if
the injury is not severe, it is important to meet with Mrs. Baranofsky
to devise a return-to-school plan that meets the needs of both the
student and the school. Thank you for your assistance with this program.

It is a good time to remind both students and parents about the importance of having breakfast before school. In order to be successful, children need to come to school ready to learn. If they are hungry, learning will be impaired. Every day, I see several students in the Health Office with complaints of hunger, dizziness, headache, stomach-ache and fatigue. The students are offered a snack here, but this results in time lost from classes. Some children do not eat breakfast before school for a variety of reasons. If this is the situation for your child, it would be helpful if you would provide a daily snack that would carry him/her until lunchtime. | ||||||||||||||||||
Physical Examinations --to Enter 7th Grade next Fall |

Attention all 6th-Grade parents
All
students entering 7th grade next fall, in the state of Massachusetts,
are required to have a Physical Exam completed within the 12-months preceding the start of school. This exam record is due in the Hale Health Office by the START of school in September.
Call the Nurse's Office if your child's
appointment does not fit into that schedule.

| Documentation of these immunizations must be received prior to the beginning of school. Please send copies of the Physical Exam and Immunization Records to the Nurse's Office as soon as your child has completed these requirements. Do not wait until school starts in September, your child will not be allowed to begin seventh grade until all records have been received and reviewed. |

| The Nashoba Regional School District is looking for nurses to substitute on an as-needed basis in all of our schools. Orientation will be provided to ensure a safe comfort level for the substitute nurse. Substitutes may choose to work at any of the schools in the district.
